Commonwealth Care Alliance’s Community Health Services (CHS) Department is responsible for addressing health related social needs (HRSN) and social determinants of health (SDOH) for enrollees in One Care (ICO) and Senior Care Options (SCO) plans. Community Health Workers (CHWs) are key members of interdisciplinary care management teams and complement and support each enrollee’s needs, goals, and preferences. The Community Health Worker I is a CHW within the Community Health Services Department who supports a panel of enrollees to overcome barriers to care and meaningful engagement related to social needs. CHW Is will utilize evidence-based resources, knowledge of community-based care and local supports, recovery-centered care skills, and health coaching / education techniques to connect enrollees with community supports and health plan benefits. CHW Is help enrollees access the most appropriate care and resources for their needs, including long-term services and supports, civic and governmental programs, and social engagement opportunities. The CHW I focuses on reducing gaps in preventive care, optimizing enrollees’ formal and informal supports, and promoting engagement with medical, behavioral health, and substance use providers as appropriate. CHW Is contribute to appropriate service utilization by identifying SDOH factors that may contribute to high frequency of emergency services and collaborating with the enrollee to identify more appropriate and efficacious supports that empower enrollees, promote community independence, and meet enrollee needs and preferences. CHW Is support CCA’s current and future Community Health Services programs and initiatives as appropriate based on experience, specialization, or expertise.
The CHW I reports to the Community Health Services Manager.
