Registered Nurse, Hospice : On-call

Salary : $85,000.00 - $94,000.00 per year
Essential Duties and Responsibilities :
- Provide direct patient care during after hours, addressing both scheduled and unscheduled needs.
- Respond to patient and caregiver needs via telephone, home visits, or facility visits for assessments, symptom management, and treatment.
- Rotate on-call responsibilities on a 7 on / 7 off schedule, starting each on-call period at 4 : 30 PM and covering all hours, including entire weekends.
- Work collaboratively with a backup Registered Nurse Case Manager (RNCM), a Supportive Care Team Member, and an on-call Administrator to ensure comprehensive patient care.
- Maintain accurate and detailed patient records in accordance with regulatory standards and organizational policies.
- Teach patients, families, and caregivers proper end-of-life care techniques and measures.
- Participate in interdisciplinary team meetings to review cases and coordinate plans that align with patient care goals.
Minimum Position Qualifications :
Bachelor's or Associate's degree in Nursing.Current and active CPR certification.Minimum of 1 year of experience in nursing, preferably in hospice or palliative care settings.Strong organizational skills and ability to multitask effectively in a dynamic environment.Excellent verbal and written communication skills.Proficiency in Microsoft Office applications and familiar with medical documentation software.Preferred Qualifications :
2-5 years of experience in hospice, palliative care, or similar field preferred.Experience with Medicare Hospice Certification and CHAP guidelines.Strong knowledge of hospice care, services provided by an interdisciplinary group, and end-of-life care standards.Advanced problem-solving skills and ability to analyze complex situations.High attention to detail and accuracy, particularly in patient documentation and compliance with health regulations.Skills :
