Registered Nurse, Hospice : On-call

Join our compassionate hospice team as an On-call Registered Nurse (RN) and contribute to our mission to provide exceptional end-of-life care during after-hours. This role suits a dedicated nursing professional ready to support patient needs during evenings and weekends, ensuring continuous and comprehensive care outside normal business hours. Salary : $85,000.00 - $94,000.00 per year

Essential Duties and Responsibilities :

Provide direct patient care during after hours, addressing both scheduled and unscheduled needs.
Respond to patient and caregiver needs via telephone, home visits, or facility visits for assessments, symptom management, and treatment.
Rotate on-call responsibilities on a 7 on / 7 off schedule, starting each on-call period at 4 : 30 PM and covering all hours, including entire weekends.
Work collaboratively with a backup Registered Nurse Case Manager (RNCM), a Supportive Care Team Member, and an on-call Administrator to ensure comprehensive patient care.
Maintain accurate and detailed patient records in accordance with regulatory standards and organizational policies.
Teach patients, families, and caregivers proper end-of-life care techniques and measures.
Participate in interdisciplinary team meetings to review cases and coordinate plans that align with patient care goals.

Minimum Position Qualifications :

Bachelor's or Associate's degree in Nursing.

Current and active CPR certification.

Minimum of 1 year of experience in nursing, preferably in hospice or palliative care settings.

Strong organizational skills and ability to multitask effectively in a dynamic environment.

Excellent verbal and written communication skills.

Proficiency in Microsoft Office applications and familiar with medical documentation software.

Preferred Qualifications :

2-5 years of experience in hospice, palliative care, or similar field preferred.

Experience with Medicare Hospice Certification and CHAP guidelines.

Strong knowledge of hospice care, services provided by an interdisciplinary group, and end-of-life care standards.

Advanced problem-solving skills and ability to analyze complex situations.

High attention to detail and accuracy, particularly in patient documentation and compliance with health regulations.

Skills :

Strong empathy and interpersonal skills to interact effectively with patients and families experiencing grief.

Exceptional clinical skills in assessing, planning, implementing, and evaluating nursing care.

Ability to work independently and as part of a team to provide effective and sensitive patient care.

Adept in managing emergency situations efficiently and effectively.
