Eastchester Family Services is a nationally accredited, HRSA NHSC-approved, community-based organization with offices in Atlanta, Cartersville and Douglasville. Our agency is designed to meet the diverse needs of todays families by effectively integrating behavioral health and primary care services to improve clinical outcomes, access to health care, and overall quality of life for our clients and their families.

We are seeking a compassionate and skilled Family Nurse Practitioner (FNP) to join our agency operating under an integrated services model. The FNP will provide holistic, patient-centered care that addresses both physical and behavioral health needs. This role involves collaboration with a multidisciplinary teamincluding psychiatrists, therapists, care coordinators, and primary care providersto ensure comprehensive treatment and improved health outcomes for patients across the lifespan.

• This is an in-person, employee role. Candidates must reside in Georgia and be able to commute to the office or designated meeting place for clients. Please expect that all services will be provided in-person. The person selected for this role will work closely (on-site) with Eastchester's community partners in Cobb, Fulton and Douglas counties.
