Registered Nurse (Health Educator)

Job Description

The ideal candidate will be a compassionate and dedicated Registered Nurse (RN) joining our team at Eisenhower Center, a Specialized Residential setting focused on the rehabilitation and habilitation of individuals with Traumatic Brain Injury (TBI), Intellectual and Developmental Disabilities (I / DD), Mental Illness (MI), and Autism Spectrum Disorder (ASD). The RN / Health Educator will provide high-quality care, support, and medical oversight to our residents, ensuring their health and well-being.

Role and Responsibilities

• Provide comprehensive nursing care to residents with TBI, I / DD, MI, and ASD.
• Administer medication and treatments as prescribed by healthcare providers.
• Monitor and assess residents' health status, identifying any changes or concerns.
• Develop and implement individualized care plans in collaboration with the facility treatment team.
• Educate residents and facility direct care staff about health conditions, treatments, and self-care strategies.
• Maintain accurate and up-to-date medical records and documentation.
• Coordinate with other healthcare professionals, including physicians, therapists, and social workers, to ensure holistic care.
• Participate in care team meetings and contribute to the development of care strategies.
• Ensure compliance with all relevant regulations, policies, and procedures.
• Provide emotional support and advocacy for residents.
• Assist with daily living activities and promote a safe and supportive environment.
• Traveling between facilities in Manchester, Ann Arbor and Ypsilanti
