LeeKO Home Health LLC provides exceptional home health services helping patients regain independence and improve quality of life. We are seeking a skilled Occupational Therapist (OT) and Occupational Therapist Assistant (COTA) to join our team.

Responsibilities OT :

  • Deliver in-home occupational therapy per physician orders.
  • Develop treatment plans to improve daily living skills.
  • Educate patients and caregivers on exercises and mobility.
  • Document visits in Synergy EMR.
  • Collaborate with interdisciplinary team members.

Responsibilities COTA : (COTA)

  • Support OT in patient care plans.
  • Document progress accurately.
  • Strong documentation skills.
  • Document visits in Synergy EMR.

    • Requirements

    Qualifications OT :

  • Florida OT license.
  • 1 year clinical experience (home health preferred).
  • Strong communication documentation and organizational skills.
  • Reliable transportation.

    • Qualifications COTA :

  • Florida COTA license.
