Job Description :

The Case Manager manages clients eligible for In-Home programs within the service area of Utah, Wasatch, and Summit Counties. This position may facilitate the transition from long-term care facilities to less restrictive, community-based facilities. The Case Manager possesses a strong knowledge of community and economic issues related to human & social services, and an understanding of health, nutrition, and supportive services relating to aging adults who meet nursing home level of care.

Essential Duties & Responsibilities :

• Oversees case management for eligible clients
• Coordinates with RN to ensure an appropriate level of service to clients
• Identifies appropriate community resources to provide additional support
• Monitors care plans and coordinates with designated service providers
• Processes intakes for in-home service programs
• Facilitates the transition from long-term care facilities to less restrictive, community-based settings as needed
• Provides information and referrals; serves as an active client advocate
• Ensures safe living environments for clients
• Travels throughout MAG's service area (Summit, Utah, and Wasatch Counties)
• Other duties as assigned.
