Nurse Practitioner

Sage Elevate is a startup outpatient / ambulatory behavioral health treatment center located in Tampa, FL.

We are looking for a full-time autonomous nurse practitioner to join our team.

From a quick start to upfront pricing and financial assistance, to compassionate care by our clinical experts in a serene environment, patients experience treatment like never before.  As the face of Sage Elevate, you will be the key provider, responsible for Behavioral Health   medication administration, response management, and post-care for patients.

In addition, the APRN will monitor patients on our premises during treatment and provide the necessary education on treatment regimen, condition management, and potential side-effects as appropriate.

The APRN is an integral member of a multidisciplinary team and will work closely with intake and clinical staff.

This position has the opportunity to grow and can include more hours as needed.

Major Responsibilities :

• Provide medication management and monitor patient responses to treatment.
• Collaborate with other healthcare professionals to coordinate care for patients with complex needs.
• Chart / schedule review .
• Determine appropriateness of patient and medication administration that day.
• Reach out to referring providers as necessary for questions / concerns related to patient condition and medication administration.   Maintain a safe environment for patients during observation periods as well monitoring clinically per Sage Elevate protocols.
• Participates in specialized one on one training of other clinical staff as needed.
