job_description.job_card.job_descriptionCase Manager Bonus (weekly) Health Insurance Free Dental Insurance Free Short-term Disability Free Life Insurance Paid Time Off 6 Paid Holidays Weekly Pay No Nights & Weekends (except on-call rotation) PPE Provided for All Staff Essential Job Functions As an RN Case Manager , you will play a vital role in providing patient-centered care while ensuring clients receive the support they need in their homes and communities. 🔹 Conduct comprehensive assessments and develop individualized care plans. 🔹 Provide nursing care, medication administration, and health education . 🔹 Monitor and reassess patient needs , adjusting care plans accordingly. 🔹 Collaborate with physicians, families, and support staff . 🔹 Supervise PCA / home health aides and mentor new RN Case Managers. 🔹 Maintain accurate documentation and fulfill certification / recertification requirements. 🔹 Participate in on-call duties and quality improvement initiatives . Requirements Active Registered Nurse (RN) License Two-Step TB Test (we provide testing if needed) DHS Criminal Background Clearance (we provide screening) Valid Driver’s License & Auto Insurance Ready to Make a Difference?
