Shift : Saturday-Sunday 12 : 00pm-4 : 00pm

Responsibilities

• Provides clinical consultative services to patients and therapeutic recommendations to nurses physicians and other hospice personnel
• Conducts and evaluates medication profiles / histories as well as assess compliance
• Suggests modifications to optimize medication regimens using sound clinical pharmacy principles accounting for pharmacodynamic and pharmacokinetic variations in drug absorption distribution metabolism and elimination to achieve desired therapeutic outcomes
• Answers customer questions and helps resolve customer issues in a timely and professional manner
• Enters and retrieves data in the pharmacy system
• Transcribes verifies and fills medication orders
• Maintains proper storage and security conditions for medications
• Contributes to the maintenance of a productive and error-free work environment by keeping pharmacy workspaces neat and orderly
