  • The pay for this position is : $35.10 / hour
  • PRINCIPAL DUTIES AND RESPONSIBILITIES :

    Resource Advocacy :

  • Provide new participant overview / orientation of BYC / RA services, intake session, and complete
  • privacy forms.

  • Meet with youth participants for one-on-one resource advocacy sessions via walk-in and
  • scheduled sessions

  • Assess and enroll eligible youth participants into case management services and develop a youthcentered
  • service plan.

  • Support youth in achieving individualized case management goals by linking participants to
  • and / or supporting systems navigation of healthcare, housing programs, workforce development,

    public benefits, and other support services.

  • Create and enact discharge plans for clients; providing follow-up at least every three months.
  • Maintain regular contact with youth participants and reassess goals as necessary.
  • Document all service provision as progress notes and referrals through the electronic health
  • record systems (HMIS, Centricity) and other internal tracking systems (Excel). All documentation

    should comply with agency privacy and confidentiality practices.

  • Assist manager of team with program audits and funders reporting
  • Coordinator Responsibilities :

  • Assist with the briefing and debriefing of Drop In and Resource Advocacy team huddles by taking
  • part in alternating tasks such as facilitating, time-keeping, and healing.

  • Lead development and implementation of group based life-skills programming targeting teens
  • (12-19) and young adults (20-24) within the Drop In milieu setting and offsite at partner

    agencies / schools.

  • Collaborate with other BYC Team Leads and Howard Brown Case Managers to ensure the
  • development and execution of age-out linkage plans for youth 24 years and older

  • Support Community Meeting by gathering resources for Resource of the Week, facilitation, and
  • other tasks as needed.

  • Support Resource Advocacy Manager in training and supervision of Resource Advocates as well
  • as team volunteers and interns.

  • Attend, support, & facilitate (on rotating basis) weekly CHAT meetings, RA meetings, self-care
  • sessions, joint team meetings, and BYC staff meetings.

  • Attend other trainings, workshops, and off-site visits to other programs as necessary
  • Attend all necessary meetings and collaborations for housing opportunities
  • Monitor team supplies and inventory through distribution and order processes
  • Support other off-site HBH drop-in services as needed
  • Proactive relationship building with collaborative partners
  • Others duties as assigned.
  • QUALIFICATIONS, KNOWLEDGE, SKILL REQUIRED :

    Required :

  • High School Diploma / GED
  • Must be highly organized, a strong communicator, and demonstrate the ability to work effectively
  • with diverse populations

  • Previous case management experience required.
  • Bilingual Spanish-English highly preferred.
  • Previous trainings on harm-reduction, youth work, and providing trans and gender-nonconforming-
  • affirming care is also highly regarded.

  • Must have previous experienced working with LGBTQ community, youth experiencing
  • homelessness, or marginalized communities.

    ADA SPECIFICATIONS

  • Requires ability to speak audibly and listen actively.
  • Requires ability to use computers, telephones and other office equipment.
  • Requires ability to sit for extended periods of time.
  • May require occasional bending and lifting up to 25 pounds.
  • May require periodic travel.
