Job Details

What You Will Be Doing

Field RN seeing patients in their homes doing OASIS admissions, assessments, providing clinical care as needed, resumptions, discharges

Provide patient care coordination working with an interdisciplinary healthcare team

Clinical care encompasses wound care, medication administration, injections

Provide patient teaching and education

Skills You Need

Active and unrestricted NYS RN license

Home Health experience preferred but not required
