Eden Health is hiring a Community Liaison Coordinator to support our business development efforts in the Portland Metro Area. This position is focused on census growth brand recognition and reputation in our service areas for our Home Health and Hospice service lines.

Job Summary :
- Coordinates marketing team efforts to increase referrals from new and established accounts and increase brand awareness.
- Assists Executive Director in establishing marketing goals / targets for team members.
- Assists Executive Director in performance evaluation and / or disciplinary action of marketing staff.
- Provides support and training for marketing staff.
- Identifies opportunities for Agency growth and develops / executes plan to achieve same.
- Coordinates / provides education to the community referral sources and facilities.
- Represents / promotes Eden Health lines of business (Home Health Hospice Home Care) in community relations activities.
- Responsible for identifying developing and maintaining relationships with referral sources including but not limited to Hospitals Long Term Care facilities rehabilitation centers and physician practices.
- Works closely with Agency administrator in creating and implementing a marketing and sales plan to increase brand awareness.
- Works with Agency administrator in developing relationships with accountable care organizations bundled payment initiatives etc.
- Explains Eden Health programs to patients family members and other referral sources in an accurate complete and thorough manner.
- Provides accurate and timely documentation of marketing team activities back to the Agency as requested.
- Follows-up on referrals for evaluation and / or admission to Eden Health programs.
- Attends and participates in in-services staff meetings QAPI meetings and case conferences as appropriate.
- Ability to communicate effectively with patients coworkers and other healthcare professionals.
- Completes documentation in an accurate timely and legible manner.
- Other duties as assigned.
Education / Experience Requirements :
Clinical background preferred but not required.Knowledge of duties and functions of the home health Agency and its relationship to the healthcare community.1-2 years experience in the home health or healthcare industryCareer Benefits :
