Registered Nurse, ICU Licenses : RN-CA Certifications : BLS-AHA(Copy needed)

CERT-NIH Stroke Scale(Copy needed) SSN Required DOB Required Work History : Should not be more than 90 days of gap in entire work history References : 1 Reference in entire work history NIHSS, DCI / BVP
