Description

US Heart and Vascular is in need of a Full-time Intake and Referral Coordinator to join our team at Samual Family Cardiology in Spring TX

Responsibilities :

• Receive review and process cardiovascular patient referrals
• Verify insurance and benefits
• Maintain accurate and timely referral logs and dashboards for tracking.
• Flag high-risk referrals
• Follow up with referring physicians regarding appointments or inability to reach patients
• Document all referral activities within the EMR / EHR accurately and promptly.
• Support reporting on referral volume.
• Work closely with Business Development team Practice Manager and frontline staff to ensure excellent patient service and maintain referral relationships
• Participate in team huddles to review pending referrals
• Provide feedback from patients and referring providers to improve workflows and communication.
