Position Summary

The Occupational Therapist (OT) provides medically prescribed, goal-oriented therapy in accordance with the Physician's Plan of Care , company policies, and professional standards of practice.

You will collaborate with an interdisciplinary team to coordinate care and ensure the highest level of patient outcomes.

Key Responsibilities

Develop and implement individualized patient occupational therapy plans of care

Conduct in-home evaluations and assessments

Communicate with physicians, nurses, and care team members as needed

Provide education and training to patients, families, and caregivers

Participate in interdisciplinary team meetings to coordinate and review care plans

Ensure timely, accurate, and compliant electronic documentation

Apply agency policies and adhere to all state and federal regulatory standards

Continuously evaluate patients' progress and update care plans accordingly

Requirements

Current Occupational Therapist (OT) license in the State of Florida (in good standing)

Knowledge of occupational therapy principles, methods, and best practices

Excellent communication, clinical documentation, and time management skills

Strong critical thinking and problem-solving abilities

Ability to work independently and collaborate effectively within a multidisciplinary team

Minimum 1 year of home health or multidisciplinary OT experience preferred
