Summary

This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific eligibility requirements per VHA policy and submit your EDRP application within four months of appointment. Program Approval, award amount (up to $200,000) & eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after review of the EDRP application. Former EDRP participants ineligible to apply.

This Registered Nurse - Community Living Center (CLC) position is located at the William S. Middleton Memorial Veterans Hospital in Madison, WI. The CLC is a sub-acute, short stay, extended care environment with a 26-bed blended care environment consisting of Transitional, Hospice / Palliative, and Rehabilitation levels of care. The average length of stay (ALOS) is less than 30 days. The adult / geriatric Veteran population served includes individuals who may be functionally impaired with a wide variety of diagnostic illnesses not limited to cancer, dementia, diabetes, substance abuse, pulmonary, cardiac, digestive, renal, hepatic, and hematologic disorders. CLC Nursing personnel are intricately involved in development, implementation, and evaluation of interdisciplinary care. We work together as a team to provide the highest quality of care with a veteran-centered approach.

The Registered Nurse - Community Living Center is responsible and accountable for all elements of the nursing process when providing and / or supervising direct patient care. Assesses, plans, implements, and evaluates care based on age specific components. Assumes responsibility for the coordination of care focused on patient education, self-management, and customer satisfaction throughout the continuum of care. Administers medications and procedures per established policies and guidelines. Influences care outcomes by collaborating with members of the interdisciplinary team. Executes position responsibilities that demonstrate leadership, experience, and creative approaches to management of complex client care.

The Registered Nurse - Community Living Center executes position responsibilities that demonstrate leadership, experience, and creative approaches to management of complex resident care. Collaborates in the development, implementation, evaluation, and revision of policies, procedures, and / or guidelines. Demonstrates performance and leadership that is broad enough to improve the care for a group of residents. Provides peers with formal or informal constructive feedback for improvement. Supports colleagues and other nurses through knowledge sharing to provide safe, quality nursing care. Fosters a safe and supportive environment conducive to the professional development of health care professionals. Evaluates outcomes of evidence-based decisions and practice changes for individuals, groups, and populations. Responsible for the documented outcomes at the program or service level. The CLC RN collaborates with the Minimum Data Set (MDS) coordinator to ensure accuracy of the Resource Utilization Group (RUG) score.

An Associate Degree (ADN) or Diploma in Nursing, with no additional professional nursing required.

An ADN or Diploma in Nursing AND 1 year of specialized nursing experience equivalent to Nurse I, Level 1 ; OR a Bachelor of Science in Nursing (BSN) with no additional professional nursing experience required.

Nurse I, Level III

Demonstrates proficiency in practice based on conscious and deliberate planning. Self-directed in goal setting for managing complex client situations.
