We are seeking a motivated and detail-oriented LVN Home Health Care Coordinator to join our administrative and clinical coordination team.

Position Summary :

The Home Health Care Coordinator (LVN) plays a key role in coordinating patient care, communicating with clinicians and physicians, managing authorizations, scheduling visits, and ensuring documentation compliance. This position requires a strong clinical understanding, high attention to detail, and excellent communication skills.

Key Responsibilities :

• Coordinate and schedule Skilled Nursing and Therapy visits based on patient needs and physician orders.
• Serve as the main point of contact between clinicians, patients, families, and providers.
• Review and assist with clinical documents to ensure completeness and compliance.
• Track SOC, ROC, Recertification, DC, and ordered visits to ensure timely service delivery.
• Monitor visit notes, wound photo attachments, and route sheet submissions to meet the 48-hour documentation policy.
• Assist with obtaining, submitting, and tracking insurance authorizations (Kaiser, LA Care, Carelon, etc.).
• Receive and process new referrals; ensure timely follow-up and initiation of care.
• Update and maintain accurate records within the EMR system (MyUnity).
• Support daily office workflow and coordination needs.
