Social Worker – Behavioral Health (BHC)

Job Description :

We are seeking an experienced  Social Worker  to join a Behavioral Health Center team providing compassionate and comprehensive care to patients. The ideal candidate will have strong clinical skills, a background in social work within behavioral health settings, and the ability to support patients and families through assessment, counseling, and care coordination.

Responsibilities :

Conduct psychosocial assessments and develop treatment plans.

Provide crisis intervention and therapeutic support to patients and families.

Collaborate with interdisciplinary teams to ensure coordinated patient care.

Document interventions and progress notes accurately and timely.

Support discharge planning and connect patients to community resources.

Job Requirements

Active  California LCSW  or  MSW  (Licensed or License-eligible).

Minimum  1–2 years  of recent experience in behavioral health or medical social work.

Strong communication and clinical documentation skills.

Experience in inpatient or outpatient behavioral health preferred.
