Additional Information About the Role

Alton Memorial Hospital is looking for a Full Time RN Case Manager. This position will be on-site and be assigned to Hospitalist care teams.

Monday - Friday 8am - 4 : 30pm; days only

No weekend requirement

Starting as a land gift to the community from the Smith family, Alton Memorial today is a full-service acute care hospital. Alton Memorial offers patients a variety of inpatient and outpatient services, including surgery services, medical imaging, interventional and diagnostic heart services, cancer care, rehabilitation, 24-hour emergency care, ambulance services and more.

Case Management, Utilization Review, Social Services : Provides admission and continued stay reviews on all patients for the purpose of compliance and the proper utilization of resources. Provides social service resources to patient's and their families.

Role Purpose

Coordinates, negotiates, procures and manages the care of patients to facilitate achievement of quality cost-effective outcomes. Works collaboratively with physicians and interdisciplinary staff, internal and external to the organization, to identify and resolve issues. Performs and provides consultation regarding Utilization Management and Performance Improvement programs in order to obtain optimum value for the patient and the reimbursement source.
