Job Summary
The Social Work Associate (SWA) works with high risk mentally ill and / or substance abusing clients living in permanent supportive housing to prevent homelessness and link them to appropriate outpatient services. Through community outreach, the SWA assists with providing individual intensive clinical case management, counseling, crisis intervention, 5150 assessment, linkage to substance abuse treatment, psycho education, intake, assessment, and individualized service planning to their caseload.
The SWA coordinates closely with hotel support services and property management staff, as well as Roving Team members and other outside providers, to develop successful treatment strategies, treatment advocacy and coordination with other agencies and natural supports. The SWA tracks client contacts, treatment plans, assessments, evictions and makes appropriate referrals. The SWA maintains client records according to California, City and County requirements; and participate in an interdisciplinary treatment team.
The final salary and offer components are subject to additional approvals based on UC policy.
Your placement within the salary range is dependent on a number of factors including your work experience and internal equity within this position classification at UCSF. For positions that are represented by a labor union, placement within the salary range will be guided by the rules in the collective bargaining agreement.
The salary range for this position is $29.79 - $38.74 (Hourly Rate).
