Summary : The University of North Carolina (UNC) Medical Center Department of Pharmacy has an opening for a highly motivated pharmacist seeking to join our team of clinical oncology pharmacists in Chapel Hill, NC. The individual in this position will form part of the breast oncology pharmacy model to support patients seen through the UNC Lineberger Comprehensive Cancer Center. This individual will gain extensive experience in comprehensive medication therapy management of breast oncology patients. The position offers the opportunity to work in a multidisciplinary team through collaboration with physicians, APPs, nurses, clinical pharmacists, and the oncology operational pharmacy team to provide comprehensive care to a diverse patient population within the breast oncology space. The position offers the opportunity to develop preceptorship and research skills through pharmacy student and PGY1 / PGY2 resident precepting and mentorship.

This individual will report to the Clinical Manager of the Hematology / Oncology Service Line and work closely with the Hematology / Oncology Clinical and Central Specialists as well as the Specialty Pharmacists within the UNC Carolina Care Pharmacy Care Network.

The individual in this position will provide comprehensive pharmacist care, disease management, and specialty medication monitoring and management services to patients seen through the breast oncology clinical service. Primary responsibilities include, but are not limited to, providing comprehensive pharmacist care to patients, collaborating closely with Acute and Ambulatory Clinical Pharmacists to ensure continuity of care between acute and ambulatory services, and leading quality improvement and outcomes initiatives.

Clinical Service Responsibilities

• Manage a clinic in collaboration with physicians, advanced practice providers, and clinical pharmacist practitioners by performing medication therapy management
• Respond to drug information questions from the medical staff, nursing staff, or pharmacy colleagues
• Function as a limited oncology prescribing practitioner (LOPP) to clinically manage chemotherapy treatment plans
• Evaluate, monitor, and redesign medication regimens to ensure positive patient outcomes
• Educate patients and their caregivers about their chemotherapy plan
• Complete therapeutic drug monitoring and manage chemotherapy toxicities
• Coordinate medication-related aspects of transitional care
• Serve as a liaison between the breast oncology team, UNC Medication Assistance Program, and specialty pharmacies (including UNC Shared Services Center Pharmacy) to help address medication access barriers and ensure timely processing and delivery of specialty medications and assure all activities are coordinated and consistent
