Responsibilities
Provides clinical expertise in the nursing management of the patients, documentation, and care planningProcesses start of care workflow once start of care visit has been completedParticipates in IDGReviews visits plotted on SOC or Re-cert visit and adjusts as necessaryMaintains orders in HCHBManages admissions, deaths, discharges as well as assists with these to make sure proper documentation is completedAssists in data collection for compliance, quality assurance, implementation of nursing policies and procedures, and staff development as assignedUpholds effective communication with referral sources to ensure smooth transition of admissionsQualifications
Associate degree in Nursing with current licenseThree years practice as a Registered NursePrevious Hospice experience preferredKnowledge of the Hospice Philosophy of CareAbility to use Microsoft Word, Excel, Outlook, and other software programs
