Responsibilities :

Provide high-quality, patient-centered care to hospice patients and their families

Conduct initial and ongoing assessments to identify patient needs and develop individualized care plans

Order and interpret diagnostic tests, prescribe medications, and provide treatments as needed

Collaborate with interdisciplinary team members to ensure comprehensive care

Educate patients and families on disease management, symptom control, and end-of-life care

Develop and maintain relationships with patients, families, and healthcare providers

Maintain accurate and timely records of patient care and communication

Requirements :

Master's degree in Nursing (MSN) or Doctor of Nursing Practice (DNP)

Active Nurse Practitioner (NP) license in the state of Oklahoma

Minimum 0 years of experience in hospice or palliative care

Strong clinical skills and knowledge of hospice and palliative care principles

Excellent communication, interpersonal, and organizational skills

Ability to work in a fast-paced, dynamic environment
