Job Summary : Acclaim Multi-Specialty Group has an exciting opportunity for experienced Advanced Practice Providers to join the Community Health / Primary Care Float Team. The Float Team supports 11 JPS Patient-Centered Medical Homes throughout Tarrant County, collaborating with Family Medicine, Internal Medicine, Pediatrics, and Advanced Practice Professionals.

We are looking for providers who thrive in dynamic environments, enjoy a challenge, and can seamlessly transition between different clinic settings. If you love working in a fast-paced outpatient setting and are enthusiastic about serving the residents of Tarrant County, this position is for you!

Essential Duties and Responsibilities :

• Work under physician supervision to provide comprehensive outpatient care.
• Perform physical exams, diagnose illnesses, order and interpret tests, and develop treatment plans.
• Educate patients on preventative care and prescribed treatments.
• Monitor care guidelines to ensure compliance with JPS Health Network and Acclaim Physician Group standards.
• Make appropriate patient referrals as necessary.
• Maintain a collaborative environment with physicians, clinical staff, and support teams.
• Support academic, educational, and research initiatives.
• Participate in Performance Improvement Programs and Peer Review meetings.
• Other duties as assigned.
