Job Title : Case Manager - Transitional Housing Program
Exempt / Non-exempt : NON-EXEMPT
Immediate Supervisor : Program Manager - Transitional Housing Program

Essential Functions :
1. Primarily responsible for developing concise and articulate individual program case plans and required funder intake documentation for all residents on caseload in prescribed time frame. Connects youth residents to appropriate support services including counseling education and vocational training healthcare and substance abuse treatment. Shares on call 24 / 7 duties with Program Manager and Case Managers across the program for any major emergencies / crisis. Staff members are expected to be reachable via cell phone during on-call weeks and respond on site within one hour if needed. May transport youth in their own vehicle or agency vehicle. May work extended hours evenings and weekends.
2. Use a trauma informed approach that emphasizes understanding about how oppressive and unjust systems impact our young people the Senior Case Manager builds positive and supportive relationships with residents in transitional housing programs.
3. Collaborate with residents to develop individualized service plans that address housing stability education employment life skills and mental health needs.
4. Maintain accurate and up-to-date case records progress notes and documentation in compliance with program and regulatory requirements.
5. Prepare reports on program outcomes client progress and service utilization.
6. Handle confidential information with discretion.
7. Prepare documents and files for audit purposes ensuring accurate and timely submissions.
8. Provide guidance on financial literacy budgeting and accessing public benefits.
9. Handles all crisis situations with sensitivity and experience consulting with supervisory chain as necessary.
10. May run reports in the Homeless Management Information System (HMIS).
11. Perform other duties as assigned.
Knowledge and Ability Requirements :
Education and Experience :
Minimum of three to five years of experience in case management youth services or a related role with a focus on serving homeless or at-risk youth populations. Bachelors degree in Social Work Psychology Counseling or a related field preferred.

To perform this job successfully an individual must be able to perform the essential duties listed in the job description satisfactorily. While performing the duties of this job the employee is regularly required to talk or hear. The employee is frequently required to sit. The employee is occasionally required to stand; walk; use hands to finger handle or feel objects or controls; reach with hands and arms. Capable of operating standard office equipment including a computer and keyboard calculator typical business machines such as a fax machine and copier. The employee must occasionally lift and / or move up to 20-30 pounds. Specific vision abilities required by this job include close vision distance vision and the ability to adjust focus. The employee is required to have valid drivers license auto insurance and must be able to safely operate a motor vehicle. The work environment characteristics include a noise level which is usually low to moderate.

Communication / Interpersonal Skills : Consistently conveys clear accurate timely and respectful communication appropriate for the situation. Informs appropriate management staff when incident occurs or a crisis situation. Maintains professional boundaries with clients / residents staff interns volunteers and vendors. Able to successfully share skills and concepts with others and works effectively with others under time and environmental pressures.
Safety and Agency Resource Utilization : Demonstrates sound judgement and foresight in safety awareness and in daily work. Notifies in a timely manner the supervisory chain of any incidents or circumstances that need attention. Report all suspicions or allegations of abuse immediately to their supervisor. Ensures appropriate utilization of materials and resources and minimizes waste.
