Massachusetts Licensed Community Health Worker

A company is looking for a Community Health Worker to provide care coordination to individuals with psychiatric conditions and / or substance use disorders. Key Responsibilities Provide outreach and engagement to individuals referred to the Care Coordination program Coordinate and contribute to the completion of required assessments to identify member needs Design personal crisis management plans in collaboration with team clinicians and members Required Qualifications Minimum of 3 years care management experience preferred, especially with Serious Mental Illness (SMI) and / or Substance Use Disorder (SUD) BA / BS in a human-services related field preferred; experience may substitute for academic training Preference for bi-lingual / bi-cultural applicants and those with lived experience of psychiatric conditions or substance use disorders Certified Community Health Worker (CHW) preferred but not required Must have access to a reliable personal vehicle and be at least 21 years of age
