A well-established long-term care facility is seeking a dedicated Geriatric Physician to oversee the medical care of elderly residents. This role requires a physician with strong clinical skills, a passion for geriatric medicine, and the ability to lead coordinated care in a residential or skilled nursing environment.
Key Responsibilities :
Provide comprehensive primary care to elderly residents in long-term care and skilled nursing facilities.
Conduct initial assessments and routine follow-ups, addressing acute and chronic conditions.
Lead interdisciplinary care teams, including nurses, therapists, case managers, and social workers.
Develop and monitor individualized care plans to maximize quality of life and function.
Communicate regularly with residents’ families and caregivers regarding care goals and health status.
Ensure compliance with regulatory standards (DOH, CMS) and maintain detailed documentation.
Participate in quality improvement programs and support staff education and training.
Qualifications :
Medical Degree (MD or DO) from an accredited institution.
Board Certified or Board Eligible in Geriatrics, Internal Medicine, or Family Medicine.
