Registered Nurse Case Manager

A leading acute care hospital located in southeastern New York is now searching for an experienced Registered Nurse Case Manager to join their award-winning healthcare team.

This acute care facility was founded over one hundred years ago and today boasts more than 400-beds and offers a wide variety of medical and surgical services. This state-of-the-art medical center provides a number of patient care services including behavioral health, oncology services, cardiopulmonary and vascular services, emergency and critical care, radiology services, maternity and pediatric care, wound and home care, and much more.

Reporting to the Director Utilization Review, the Registered Nurse Case Manager will oversee all care planning functions from pre-hospital to post-discharge stages. Primary responsibilities include planning and monitoring services, creating individualized care plans, collaborating with social workers and nursing teams, ensuring regulatory compliance, and adheres to hospital policies and procedures.

What’s Needed to Succeed :

• RN Registered Nurse license in the state of New York or ability to obtain
• Clinical background in an acute care setting
• Prior working experience in care coordination or similar setting
