Coordinated Family Support (CFS) Specialist

Job Title : CFS Specialist

Program : Coordinated Family Support (CFS)

Reports to : CFS Program Manager, COO

Work Location(s) : Contra Costa and Alameda Counties.

Hours : To Be Coordinated with CFS Program Manager

Pay Range : $22.00 to $25.00 an hour (Based on experience)

CFS Specialist Position Description

The Coordinated Family Supports (CFS) Specialist is responsible for providing comprehensive case management and support services to adults with intellectual and developmental disabilities who choose to remain in the family home. This role involves coordinating access to resources, facilitating service delivery, and supporting long-term planning to promote stability and independence. The CFS Specialist collaborates with families, service providers, and community agencies to ensure individualized, culturally responsive support across Contra Costa and Alameda Counties.

CFS Specialist Qualifications and Requirements Education and Experience

  • BA Degree in a human services field; OR
  • General studies with a minimum of 3 years of experience in developmental disabilities or related field; OR
  • A reasonable combination of the above education and experience requirements.

Transportation

  • Valid California driver’s license
  • Clean driving record (DMV printout is required)
  • Current vehicle registration and use of a vehicle in good running condition during work hours.
  • Valid automobile insurance for that vehicle and all passengers that meets minimum coverage requirements of the agency.

  • Willingness and ability to perform essential functions such as reception, answering phones, consultations, and computer-based tasks.
  • Ability to work collaboratively with consumers and co-workers.
  • Ability to work independently and handle multiple priorities.
  • Strong verbal and written communication skills.
  • Tuberculosis Clearance Screen.
  • Current CPR and First Aid certification.
  • Pass criminal background check through fingerprint clearance.
  • Provide documents required for I‐9 employment eligibility.
  • Essential Job Duties and Responsibilities

  • Identify and coordinate supports for individuals with I / DD to remain in the family home.
  • Conduct intake, assessment, and service planning.
  • Assist with navigation of public services (e.g., Medi-Cal, IHSS, Social Security).
  • Provide education related to diagnosis and needs.
  • Coordinate consistency and quality of services from support professionals.
  • Assist with appointment scheduling and coordination.
  • Identify transportation and backup services as needed.
  • Support long-term planning including for those with aging caregivers.
  • Deliver training to promote independence and inclusion.
  • Ensure services are culturally and linguistically appropriate.
  • Prepare and submit quarterly service progress reports.
  • Report Special Incidents and Suspected Abuse as required.
  • Report abuse within 24 hours and notify supervisors of significant client changes.
  • Perform general administrative tasks as assigned.

  • Strong organizational, record-keeping, and analytical skills.
  • Proficient in conflict resolution and community navigation.
  • Effective interpersonal and team communication.
  • Tools and Technology Used

  • Computers, phones, tablets, general office and household equipment.
  • Office 365, EHR systems, time tracking, video conferencing, LMS platforms.
  • Assistive devices such as wheelchairs, gait belts, Hoyer lifts.
  • Note : Tools used may vary depending on agency needs.

    Physical Demands

    This position requires regular travel, prolonged periods of driving, and the ability to transition between various community settings. The employee must be able to sit, stand, and walk for extended periods and occasionally lift and / or move up to 50 pounds.

  • Regular driving, home and community visits.

    Work Schedule & Travel

    Work schedules are flexible and may include evenings, weekends, and holidays. Regular travel is required to client homes, office locations, and offsite meetings or trainings.
