LPN, PAT

Description

A licensed practical nurse, or LPN, provides direct and indirect nursing care under the supervision of a registered nurse or physician and reports to the Team Lead. SPECIFIC JOB DUTIES AND RESPONSIBILITIES :

• Obtains, verifies, and updates patient information and provides support services to patients and medical staff.
• Requests, locates, sends, and receives patient medical records.
• Loads labs, medications, and other patient information into the electronic medical records (EMR).
• Assists in exam rooms.
• Assists the physician(s) with examination, diagnostic procedures, and treatments.
• Provides direct patient care as ordered.
• Answers and triages phone calls under RN or provider supervision.
• Completes insurance and disability paperwork.
• Documents all interventions, patient / family responses, medication dispensed or prescribed, and hospital data in the medical record.
• Promote wellness by providing patient education materials, communicating the physician's advice and instruction.
• Coordinates patient care by triaging patients depending on the urgency of patients' needs.
• Administers injections / medications and documents all necessary information ordered by the physician.
• Adheres to safety guidelines by maintaining equipment and instruments.
• Stock supplies.
• Prepares Patient Occurrence Report / Variance Reports as needed.

OTHER JOB REQUIREMENTS : May be required to travel to satellite clinics.
