Position Summary
Pharmacy Technicians on the Clinical Team (340B) are essential members of Waco Family Medicine's dedicated 340B Capture Team. These licensed professionals deliver proactive, concierge-level (white-glove) support to patients prescribed high-value medications. By partnering closely with WFM's Community (Retail) Pharmacy team, they ensure patients experience seamless service from outreach through refill, pickup, or delivery. Their efforts directly improve access, adherence, and health outcomes while generating critical 340B revenue that sustains WFM's mission : every refill helps fund care for neighbors in need.
Key Responsibilities
Patient Identification & Engagement
Identify patients on high-value therapies (e.g., GLP-1s, SGLT2 inhibitors, DOACs, insulins, Spiriva, Trelegy, HIV / HCV therapies) who are not filling at WFM pharmacies.
Conduct direct patient outreach via phone, text, MyChart, or in-person :
Explain the benefits of using WFM Pharmacy (affordability, delivery, integrated care).
Offer seamless prescription transfer support.
Address patient concerns related to pharmacy access, cost, or service.
Care Coordination & Service
Serve as a trusted liaison between patients, providers, and pharmacy staff.
Partner daily with the Community (Retail) Pharmacy team to :
Confirm transferred prescriptions are filled accurately and on time.
Coordinate delivery, pickup, or mail services to meet patient needs.
Resolve barriers (insurance, copays, logistics) in real time.
Assist patients with synchronization of multiple prescriptions to improve convenience and retention.
Support patients with insurance navigation, prior authorizations, or appeals as directed by the pharmacist.
Collaborate with 340B Coordinator to ensure claims are accurately captured
Adherence Monitoring & Outreach
Proactively monitor refill schedules to prevent gaps in therapy.
Contact patients ahead of due dates to confirm refill and delivery / pickup preferences.
Identify adherence barriers and escalate to the pharmacist for clinical follow-up.
Work jointly with the Community Pharmacy team to implement packaging solutions, refill reminders, or other adherence supports.
Data & Reporting
Document all patient interactions, transfers, and outcomes in Epic.
Track and report on patient conversion outcomes, adherence, and leakage trends.
Contribute data for performance dashboards and KPIs (capture rate, conversion %, refill retention).
Team Collaboration
Participate in team huddles and meetings to review capture performance and share engagement strategies.
Collaborate with the Community Pharmacy team to improve workflows and strengthen patient experience.
Provide feedback to the Director of Clinical Pharmacy regarding workflow gaps, systemic barriers, or patient insights.
Assist with onboarding and educating new patients about WFM's pharmacy services.
Qualifications
Preferred Qualifications
Active pharmacy technician license in the State of Texas.
Bachelor's / Associate degree (preferred)
Minimum 5 years of experience in retail, outpatient, or ambulatory pharmacy.
Experience with Epic or similar electronic health record systems preferred.
Strong interpersonal and communication skills with a customer-service mindset.
Highly organized, self-directed, and adaptable in a fast-paced environment.
Bilingual (English / Spanish) preferred but not required.
