About the Role

As a Speech Therapist, you will serve as a key partner in your patients' journey to recovery, working closely with them to help restore and enhance their communication, swallowing, and cognitive abilities. Your work will support their independence and safety at home, ensuring optimal function and quality of life. This role follows a patient-centered Plan of Care developed in coordination with the physician and patient / caregiver.

Qualifications :

  • Current Speech Therapy license in the state of Florida
  • Completed fingerprinting
  • Minimum of 2 years of home health experience preferred
  • Commitment to Haven's mission and values
  • Strong clinical judgment and ability to work independently

    • Key Responsibilities :

  • Follow all agency policies and procedures
  • Evaluate and develop individualized care plans for patients
  • Help patients improve or adapt to communication and swallowing difficulties
  • Participate in interdisciplinary care planning
  • Document clinical progress and outcomes in a timely manner
  • Perform OASIS assessments, including admissions, resumptions, transfers, discharges, and significant change in condition
  • Collaborate with physicians on plan of care modifications
