Model : In person

Summary : Community Health Workers, who work within the community and in health care settings, empower patients and their caregivers to improve health outcomes by providing culturally sensitive education, peer-based support, and connections to primary care and community-based social supports. This position is with NMIC but works in close partnership with New York-Presbyterian (NYP) Hospital and reports to two managers, one at NMIC and one at NYP.

SALARY / BENEFITS : NMIC offers a competitive salary based on experience and education as well as a comprehensive and generous benefits package : medical, dental, life, and disability insurance are included; paid time off, including 20 vacation days, 15 sick days, 5 personal days, and 11 agency holidays; plus the week of Christmas Eve through New Year’s Day; a 401k with agency contribution; and other benefits.

SALARY : The salary for this position is $55,000 per year.
