Job Title : Care Management Medical Assistant

Location : Buffalo, New York

Remote

Terms : Permanent Full-Time

Hourly : $18 - $21 / hr

About the Opportunity :

A healthcare organization seeking a certified Medical Assistant to join their growing staff as a new Care Management Team Member. This role will be providing remote Care Management for patients throughout various long term care facilities, under the direction of a patient’s provider.

The Long Term Care MA utilizes care management process to evaluate, identify and document patient updates within long term care facilities. Health updates are noted within MPC and Facilities EMR for the providers to review prior to a scheduled visit with patients. This work is executed on each patient on a monthly basis.

Position Responsibilities :

  • Executes Long Term Care “Pre-Provider Template”, on a monthly basis, per patient.
  • Work collaboratively with the provider teams of these long term patients.
  • Work collaboratively with facility nursing team to ascertain monthly patient visit schedule within the facility
  • Work collaboratively with facility nursing team to document when patients are moved in any way; rehab / hospitalization / back home
  • Actively maintain assigned caseload of patients
  • Properly document all patient related work within CCM Excel Workbook
  • Additional duties as assigned
  • Position reports to Care Management Manager

Required Qualifications :

  • 1 year of experience within Care Management setting or Case Coordinator either in the homecare, inpatient, physician practice or in-home case management setting
  • Highly organized and strong independent worker
  • Must be highly motivated, result-oriented with strong skills in presenting, communicating, organizing and time management skills
  • Strong organizational and interpersonal skills
  • Excellent customer service skills demonstrated by positive feedback from patients / team
  • Ability to identify problems and recommend solutions
  • Demonstrates progressive proficiency with the utilization of available computer technology, including typing skills
  • Demonstrates; leadership, communication, interpersonal and problem-solving skills
  • Experience working with Electronic Health Record (EHR)
  • Ability to sit and work at a computer for extended periods of time
  • 60 words per minute typing
  • Preferred Qualifications :

  • Bilingual in Spanish
