Nurse Care Coordinator

We are seeking a dedicated Nurse Care Coordinator to join our team. In this role, you will work under the guidance of the Case Management Manager to provide exceptional support to our members with complex health needs.

Key Responsibilities :

• Conduct comprehensive assessments including physical, medical, and psychological evaluations through telephonic interviews for high-risk patients.
• Create and implement personalized care plans that address both medical and psychosocial needs.
• Facilitate effective communication and collaboration among various providers, including medical, nursing, social, and behavioral health teams.
• Encourage patient self-management by providing educational resources and support focused on disease management, nutritional guidance, and exercise recommendations.
• Manage and track the care of members with complex conditions to ensure compliance with clinical outcomes.
• Travel to different locations as necessary to enhance patient care coordination.

Qualifications :

This position offers remote work options but may require occasional visits to our Central Health office in Austin, Texas.
