Job Opening Summary

The incumbent ensures efficient disease management efforts to improve patient outcomes and patient satisfaction. The incumbent is responsible for performing initial and ongoing needs of the patient : maintaining frequent patient contact for disease specific programs and implementing appropriate interventions as prescribed. The incumbent coordinates the development, implementation and monitoring of the patient's plan of care by collaborating with the physicians and other members of the medical care team. The incumbent identifies patient educational needs and provides or coordinates ongoing education to help improve patient health outcomes.

Job Opening Qualifications

Required Education : Bachelor's degree

Specific Course Work :

Graduate of an accredited school of nursing

Preferred Education :

Master's degree

Necessary Skills :

• Organized and self-motivated.
• Able to work independently.
• Possesses assertiveness skills and above average verbal and written communication skills.
• Knowledgeable in principles of adult education.
• Excellent interpersonal skills with patients and families, staff, physicians, other disciplines and

administration.

Required Licensure / Certifications :

Preferred Licensure / Certifications :

ACLS from the American Heart Association is required for the ICU, Emergency Department,

and Specialty areas.

Required Experience :

3-5 years of Medical / Surgical RN experience.
