Role Overview

As a Hospice Aide, you will provide essential personal care under the supervision of a registered nurse, ensuring the highest level of comfort for our patients.

What We Offer

  • Competitive Pay : $19.00-$22.00

Key Responsibilities

  • Document all care provided according to organizational policy.
  • Report any changes or deviations from the Plan of Care to the Case Manager.
  • Safely use Durable Medical Equipment.
  • Monitor and take vital signs as outlined in the Hospice Aide Plan of Care.
  • Foster collaboration among team members by understanding their roles and participating actively in staff meetings.
  • Inspect and care for patients' skin to prevent tissue breakdown and report any concerns to the Case Manager.
  • Assist with ambulation, transfers, and range of motion exercises as indicated in the care plan.
  • Provide respite support to patients' families and caregivers when on-site.
  • Engage in Quality Assurance Performance Improvement (QAPI) activities.
  • Participate in Interdisciplinary Group (IDG) meetings as needed.
  • Qualifications

  • Must meet training requirements established by the state of Arizona.
  • Minimum one year of experience as a CNA, hospice aide, or home health aide.
  • Must be at least 18 years of age.
  • A valid driver's license and an insured, reliable automobile.
  • Excellent communication skills, both verbal and written.
  • Able to work effectively both independently and as part of a team.
