Role Overview

As a Hospice Aide, you will provide essential personal care under the supervision of a registered nurse, ensuring the highest level of comfort for our patients.

What We Offer

Competitive Pay : $19.00-$22.00

Key Responsibilities

Document all care provided according to organizational policy.

Report any changes or deviations from the Plan of Care to the Case Manager.

Safely use Durable Medical Equipment.

Monitor and take vital signs as outlined in the Hospice Aide Plan of Care.

Foster collaboration among team members by understanding their roles and participating actively in staff meetings.

Inspect and care for patients' skin to prevent tissue breakdown and report any concerns to the Case Manager.

Assist with ambulation, transfers, and range of motion exercises as indicated in the care plan.

Provide respite support to patients' families and caregivers when on-site.

Engage in Quality Assurance Performance Improvement (QAPI) activities.

Participate in Interdisciplinary Group (IDG) meetings as needed.

Qualifications

Must meet training requirements established by the state of Arizona.

Minimum one year of experience as a CNA, hospice aide, or home health aide.

Must be at least 18 years of age.

A valid driver's license and an insured, reliable automobile.

Excellent communication skills, both verbal and written.

Able to work effectively both independently and as part of a team.
