Monday (12 / 23) : 8 : 00 A - 5 : 00 P

Tuesday (12 / 24) : 8 : 00 A - 12 : 00 Noon

Wednesday (12 / 25) : CLOSED

Thursday (12 / 26) : 12 Noon - 5 : 00 P

Friday (12 / 27) : 8 : 00 A - 5 : 00 P

TX DEA Required

Provider must attend orientation the day prior to their first shift. Orientation is 3 hours
