POSITION SUMMARY
The Behavioral Health Case Manager plays a vital support role on the Co-Responder team working in partnership with licensed clinicians and law enforcement officers to provide follow-up services and care coordination for individuals who have experienced a behavioral health crisis. This position focuses on outreach engagement and navigation helping individuals connect to services such as mental health treatment substance use programs housing benefits and other community-based resources. The Case Manager is not typically present during law enforcement calls but serves as a bridge between crisis response and long-term stabilization.

DUTIES AND RESPONSIBILITIES

Essential Duties & Responsibilities
- Receive and triage referrals sent to co-responder team from EPD law enforcement personnel partner agencies etc.
- Provide outreach and follow-up to individuals identified through co-responder interactions and crisis calls. These can be over the phone or in person depending on situation and needs.
- Assist clients in navigating and accessing community resources including mental health and substance use treatment primary care housing assistance transportation and public benefits.
- Build rapport with individuals who may have complex needs histories of trauma or prior system involvement.
- Accompany clients to appointments and offer hands-on support with applications referrals and transitions between service providers.
- Maintain detailed timely and confidential documentation of client contacts and service plans in accordance with HIPAA and program guidelines.
- Participate in case conferencing multidisciplinary team meetings and community outreach events.
- Track client progress identify gaps in services and advocate for additional support when needed.
- Support program evaluation by contributing to data collection and outcome reporting.
- Attend and contribute to interdisciplinary team meetings training and community engagement efforts.
Other Duties & Responsibilities
Provide behavioral health training to law enforcement officers including topics such as resource identification and navigation mental health signs and symptoms trauma-informed approaches and cultural considerations in behavioral health response.Collaborate with community partners and stakeholders to deliver outreach education and training on the co-responder model mental health crisis response and available behavioral health resources.PREPARATION AND TRAINING
Education
Bachelors degree in Social Work Psychology Human Services or related field.Work Experience
At least 2 years of direct experience in case management outreach or behavioral health support.Certifications and / or Licensures
Ability to pass background check through the Englewood Police department and the Colorado Bureau of Investigations.Required Drivers License
Colorado Drivers License must be valid.An equivalent combination of education training and relevant job experience may be substituted.
KNOWLEDGE SKILLS AND ABILITIES
Knowledge of mental health substance use and social service systems.Skills and competence to establish supportive respectful trusting relationships with persons with a history of severe or persistent mental illnesses substance use disorders and / or homelessness.Ability to work effectively in a dynamic sometimes field-based environment occasionally under stressful or time-sensitive conditions.Strong collaboration and communication skills especially in multidisciplinary settings.Excellent verbal written and interpersonal communication skills.Aptitude for detail and organizational skills.Ability to maintain a high level of integrity and confidentiality.Knowledge of Denver Metro area resources.Experience working with individuals with serious mental illness co-occurring disorders or involvement in the criminal justice system preferred.Bilingual or multilingual skills preferred.HOURLY RANGE

Position is primarily based out of the police department with regular collaboration alongside co-responders and law enforcement personnel.Includes some field-based work such as client outreach in the community home visits or accompaniment to service appointments.May involve occasional exposure to individuals in crisis or unstable environments.Work week is comprised of 10-hour shifts four days of the week.APPLICATION DEADLINE
