The Assistant Case Manager 3 works in collaboration with the Physician, Psychotherapist, consulting Psychiatrist, and other care team members to assure the appropriate level of care and coordination for outpatient services. The Assistant Case Manager reports to the Healthy Young Minds Clinical Manager. The Assistant Case Manager has frequent contact with patients, families, physicians, the interdisciplinary team, schools, ancillary services, and community resources. This position is responsible for gathering and reporting patient functioning data and metrics, entering data into the patient care registry, communicating between collaborative care team members, direct patient contact to assigned patients and family members, and direct intervention with patients and families, to include crisis intervention and de-escalation where required.

This position will be on-site in Fremont 2 days / week, on-site in Milpitas 1 day per week, and remote 2 days per week

Education : Bachelor's : BA or BS in psychology, social work, or a related field.

Typical Experience : 2 years' recent relevant experience.

Skills and Knowledge : Oral and written communication skills. Interpersonal and time management skills Ability to work effectively and prioritize tasks in a fast-paced environment with rapidly shifting priorities and competing demands. Ability to work independently with a minimum of direction. Intermediate PC skills and word processing skills required.

Pay Range is $35.35 to $45.95 / hour
