Mercy Medical Center in Aurora, IL and Holy Family Medical Center in Des Plaines, IL are actively seeking Advanced Practice Providers (NP / PA) to join our Surgical team! This role supports providers across Mercy Medical Center and Holy Family, requiring flexibility to travel between both sites. The APP will provide high-quality, patient-centered care, focusing on wound management, wound debridement, and support of general surgery workflows.

Position Details

• Perform wound assessments, ongoing wound care, and wound debridement (sharp debridement required).
• Manage acute and chronic wounds in collaboration with the General Surgery team.
• Conduct patient evaluations, develop treatment plans, and participate in interdisciplinary care planning.
• Provide pre- and post-operative surgical care, including follow-up visits and patient education.
• Document all care in the EMR in a timely, accurate, and compliant manner.
• Coordinate care across both clinical sites, ensuring consistent communication with the surgical team.
• Collaborate with physicians, nursing staff, case management, and ancillary services to optimize patient outcomes.
