We are seeking a full-time Primary Care Nurse Practitioner or Physician Assistant to join our team in Stockton, CA as part of the PACE (Program of All-Inclusive Care for the Elderly) model. This role offers a meaningful opportunity to deliver comprehensive, community-based care to older adults in both clinical and home settings, with support from an interdisciplinary care team.
Key Responsibilities :
- Provide primary and preventive care to PACE participants in clinic and at home
- Collaborate with PACE physicians and the interdisciplinary team to develop and adjust care plans
- Participate in a rotating, phone-only on-call schedule (a few nights or weekends per month)
- Ensure continuity of care and manage chronic and acute conditions
- Promote health education, disease prevention, and overall wellness
Compensation & Benefits :
