Registered Nurse – Home Health

Pay : $40-45 / hr

Shift : 8 : 00 AM – 4 : 30 PM

On-Call : One weekend per month

Mileage : Reimbursed at $0.41 per mile

Location : Pleasant View, TN

Territory : Montgomery, Cheatham, Robertson, and Sumner Counties (as needed)

Position Overview :

We are seeking a dedicated and experienced Registered Nurse (RN) to join our home health team. This role involves direct patient care in the community, supporting individuals with chronic, acute, or complex medical needs. You will play a key role in assessment, planning, implementation, and evaluation of care plans to ensure quality outcomes for the people we serve.

Key Responsibilities :

Conduct comprehensive assessments and implement individualized care plans.

Deliver nursing care and support, including wound care and IV / PICC line management.

Monitor and report changes in patient conditions to physicians and other care team members.

Coordinate care across disciplines and support case management activities.

Supervise LPNs / LVNs and ensure compliance with health and safety policies.

Maintain accurate and timely documentation, including OASIS assessments in HCHB (Homecare Homebase).

Support medication administration processes, including transcription of physician orders and medication disposal protocols.

Schedule and follow up on health-related appointments and hospital discharges.

Provide training to new employees and ensure ongoing education on health and safety topics.

Ensure all infection control and disease prevention measures are followed in the field setting.

Requirements :

Active Registered Nurse (RN) license in good standing

Experience with HCHB and OASIS documentation required

Wound care and IV / PICC line experience strongly preferred

Strong communication, time management, and critical thinking skills

Reliable transportation and willingness to travel within the assigned service area
