PRIMARY RESPONSIBILITY :
The Behavioral Health Clinician works in conjunction with the Interdisciplinary Team (IDT) at On Lok PACE at the Institute on Aging (IOA) to identify and implement behavioral health services for PACE participants.
The Behavioral Health Clinician collaborates as a member of the participants care team to identify clinical strategies for serving individuals with behavioral health needs and to ensure that behavioral treatment is integrated into the interdisciplinary person-centered care plan.

COMPENSATION :
Range : $ 130000.00 - 148000 / annual
