SUMMARY
Provides Case Management Services to program participants. Counsels’ participants and prepares them to access services and progress towards the goals identified in the Individual Service Plan. Conducts outreach to community through special events and individual encounters.
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.
Conducts outreach activities to inform eligible communities of available services; and to network with community providers to establish referral relationships. Assists in the identification of new outreach opportunities and establishes relationships with a variety of organizations that allow further communication with community members regarding the Immigrant Resource Center. Participates in the follow-up of outreach and in-reach referrals from all sources to inform and assist individuals in obtaining services.
Conducts intake, assessments, and orientations for the Immigrant Resource Center.
Develops individual plan for participants that include appropriate goals and objectives relative to abilities, interests, needs, experience, and assessments, and updates as needed. Tracks participant progress to ensure continual progression towards their goals.
Effectively conducts one on one consultation of the benefits of enrolling in various programs, and assists participants with enrollment. Assists participants with barriers to enrollment.
Assists individuals to understand and overcome barriers to service acquisition and determines the support services necessary to overcome these barriers.
Provides referrals to available services; engages in regular case management to ensure services are received.
Educates participants on the navigation through selected programs including instructions on program requirements, upcoming appointments, and services to be received.
Teaches ESL, Acculturation, Financial Literacy, Computer Literacy, and other topics as needed to program participants.
Provides oral and written interpretation and translation services as needed for participants.
Engages in individual and community advocacy efforts to increase access to quality, culturally-sensitive services for the immigrant, migrant, and refugee communities.
Maintains accurate and complete records for all program participants for the purpose of case files maintenance and program reporting.
Records all participants and other stakeholders contact (case noting).
Communicates professionally with supervisors, coworkers, program participants, community agencies, and other stakeholders involved in service provision.
Assists in developing and updating acculturation and other related training materials and curriculum. Selects or develops teaching aids such as multimedia visual aids, computer tutorials etc. to enhance participant learning.
Ability to deal effectively with difficult and on occasion, irate participants. Demonstrates ability to deescalate individuals with tact and understanding.
Conducts work in a safe and responsible manner, utilizing protective clothing and / or equipment when appropriate. Complies with all safety standards.
Responsible for awareness of and adherence to all company and safety policies and procedures.
Ensures that client confidentiality is maintained in accordance with contract, State, and Federal law including HIPAA.
Responsible to understand and support CARF accreditation standards.
Attends and participates in meetings, staff development sessions and in-service training as assigned.
Works on other projects or tasks as assigned by the Program Coordinator or Director.
SUPERVISORY RESPONSIBILITIES
This job has no supervisory responsibilities.
QUALIFICATIONS

EDUCATION and / or EXPERIENCE
Bachelor's degree (B. A.) from four-year college or university preferred and one to two years related experience; or equivalent combination of education and experience; Proficiency in Microsoft Word, PowerPoint, Excel, and Outlook; Human Services experience within the immigrant community preferred.
LANGUAGE SKILLS
Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to write routine reports and correspondence. Ability to speak effectively before groups of customers or employees of organization. Ability to communicate in a second language required (Micronesian languages -Chuukese, Marshallese etc. preferred.)
MATHEMATICAL SKILLS
Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percent and to draw and interpret bar graphs.
REASONING ABILITY
Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.
CERTIFICATES, LICENSES, REGISTRATIONS
Periodic travel is required, must have a valid driver's license and access to an insured vehicle
