job_description.job_card.job_descriptionProvide, coordinate, and direct home nursing care per physicians' orders and agency policies Supervise and evaluate Home Health Aides and Licensed Practical / Vocational Nurses Engage in home visits and periodic office meetings Manage an average of 5-10 patients per day, equivalent to 6-8 points, with 30 points weekly as the productivity expectation Handle various types of cases such as Start of Care, Wound Care, Follow-Up Visits, etc. Work one weekend a month and be on-call during scheduled weekends Cover the region of Sussex County Document all patient information and activities within 24 hours of appointments Complete a retraining course if HCHB or OASIS experience is older than six months Travel approximately 200-250 miles weekly No guaranteed hours; opportunity to float to nearby clinics if patient census is low Not currently a direct employee of any relevant health organization if applying as a traveler Local applicants accepted at the same rate

Certifications : CPR, BLS
Must-Have :
- Proficiency in HomeCare HomeBase (HCHB) and OASIS
- Reliable transportation, current driver's license, and liability insurance
- Experience in Adult Home Health or Hospice
- Ability to travel up to 75 miles from the home clinic
