As an Advanced Nurse Practitioner at Transitions Care you will be responsible for the direction and oversight of the care and treatment to hospice patients rendered by the hospice care team and provide oversight for Plan of Care and all changes to the Plan of Care. Implement the Plan of Care provisions to meet the patient's medical needs. Travel, as necessary.

Major Responsibilities

• Consult and work with the patient's attending physician.
• Assure compliance with local, state, and federal laws, Medicare regulations, and HIPAA.
• Accountable for direction and oversight of the care and treatment of palliative and hospice patients.
• Implement the plan of care to meet the patient's medical needs, assess and reevaluate the patient's medical needs on a regular basis, and manage related expenditures in a fiscally responsible manner.
• Assumes responsibility for meeting regulatory and other mandatory requirements.
• Reviews and interprets diagnostic results in a timely manner.
• Prepares and maintains clinical documentation utilizing company EHR.
• Routine visits focusing on primary care and prevention.
• Complete and document all Hospice Face to Face encounters
• Care coordination with home health, specialists, facilities, and clinical coordinators.
• Emergency responder to all on-call visits as needed for case managers and clinical coordinators.
• Routine visits focusing on symptom management and quality of life.
• Ability to utilize the necessary equipment such as BP monitor, pulse oximeter, otoscope, etc.
• Serve as a liaison and contact between the hospice program, community physicians and others engaged in health care services in order to develop and expand the number of patients served by Transitions.
• Provide direct medical care as needed to patients in their homes, nursing homes and in the hospital.
• Maintain clinical records, statistics, reports, and records for purposes of evaluation and reporting of agency activities as prescribed and in compliance with local, state, and federal laws.
