Pinnacle Home Care, Florida's largest Medicare-certified home health provider, has been delivering high-quality, patient-centered care for over two decades, and we're looking for Occupational Therapists to join our award-winning team.

Key Responsibilities

• Assess the patient's functional status (muscle function, endurance, visual coordination, communication skills, self-care, work capacity, etc.).
• Evaluate the home environment for hazards or barriers to independent living and identify necessary equipment.
• Collaborate in developing a comprehensive care plan.
• Design a treatment program with goals for improved function and communicate the plan to the referring physician and other team members.
• Teach or retrain patients in daily activities lost due to illness or injury, following agency policies.
• Fabricate splints and instruct patients on using adaptive equipment to enhance function.
• Provide training in the use of prosthetic and / or orthotic devices.
• Maintain accurate clinical records and report any changes in the patient's condition to the physician.
• Participate in in-services, case conferences, and meetings to promote coordinated care with healthcare team members.
• Work alongside physicians and discharge planners to provide health education to patients and families / caregivers.
• Identify patient and family / caregiver needs for additional services and make appropriate referrals.
• Confirm daily visit schedules to ensure seamless coordination with other staff.
• Supervise Certified Occupational Therapy Assistants in accordance with agency policies and state regulations.
