Community is expanding its footprint and that means bringing on more staff to cover our Home Health Department.

\n\nResponsibilities : \n

As a Home Health Registered Nurse, you will have the opportunity to collaborate with a team of  Home Health professionals with a passion for caring for Valley patients in the safest place to them, their home. Averaging 35-30 visits per week, you'll have the unique opportunity to focus on the continuum of care for a wide acuity of patients, including establishing a plan of care.
