Job Title : Community Health Worker (CHW)

The Community Health Worker (CHW) plays a vital frontline role in Care Ring's Community-Based Services, supporting The Bridge mobile health unit, the Food Pantry, and our Access to Care programs (Care Ring Clinic and Physicians Reach Out). This position is designed to directly address Social Determinants of Health (SDOH) by connecting individuals and families to essential health, nutrition, and social resources. The CHW builds trusted relationships within underserved communities, facilitates access to care, and empowers clients through education, advocacy, and resource navigation.
Responsibilities
Direct Client Support & Navigation
- Provide culturally competent outreach and support to clients accessing The Bridge, the Food Pantry, and Access to Care programs
- Conduct needs assessments and assists clients in navigating health and social services, including housing, transportation, employment, and benefits navigation
- Facilitate referrals to internal and external programs, ensuring follow-through and reducing barriers to care
Community Outreach & Engagement
Represent Care Ring at community events, health fairs, and outreach activities to promote services and build trustCollaborate with local organizations, faith groups, and community leaders to identify needs and expand program reachSupport targeted outreach campaigns to engage high-need populations and promote health equityProgram Integration & Coordination
Work closely with the Program Coordinator to align CHW activities with Care Ring strategic prioritiesAssist with logistics and client flow during mobile health unit visits and pantry operationsSupport volunteer engagement by modeling compassionate service and providing informal mentorshipEducation & Advocacy
Deliver health education and resource information tailored to community needs (e.g., nutrition, chronic disease prevention, benefits navigation)Advocate for clients within healthcare and social service systems, helping them overcome systemic barriersPromote self-efficacy and empowerment through coaching and goal-settingDocumentation & Reporting
Maintain accurate records of client interactions, referrals, and outcomes using approved systemsContribute to program evaluation efforts by collecting feedback and identifying trends in community needsEnsure confidentiality and compliance with HIPAA and organizational policiesQualifications and Education Requirements
High school diploma or GED requiredCommunity Health Worker (CHW) certification or successful completion of requirements within 6 months of hireMinimum 1 year experience in community outreach, case management, or health navigationDemonstrated understanding of Social Determinants of Health (SDOH) and commitment to health equityStrong interpersonal skills with ability to build rapport across diverse populationsAbility to work independently and collaboratively in dynamic environmentsProficiency in Microsoft Office and basic data entry systemsReliable transportation, valid driver's license, and current auto insurance requiredWillingness to travel locally for outreach and eventsAbility to lift up to 25 pounds and perform physical tasks related to outreach and pantry supportMust be able to work a flexible schedule, including some evenings and weekendsPreferred Skills
